Healthcare Documents
☐ Healthcare Power of Attorney (Healthcare Proxy)
- Names someone to make medical decisions if incapacitated
- Should be someone who understands their wishes
- Becomes effective when person can't decide for themselves
☐ Living Will (Advance Directive)
- Documents end-of-life care wishes
- Specifies preferences for life-sustaining treatment

☐ HIPAA Authorization
- Allows specified individuals to access medical information
- Separate from healthcare POA
